PLEASE COMPLETE THIS ONLINE REQUEST FORM AND YOUR CHILD'S AAA PERMISSION TO SKATE AND /OR JR FORM WILL BE EMAILED TO THE EMAIL PROVIDED IN THE REQUEST BELOW.

2024-2025 PTS for U18 will be issued in August 2024

1. IF YOU HAVE MORE THAN ONE CHILD PLEASE COMPLETE A SEPARATE REQUEST For each.child

2. A PTS (Permission to skate) FORM IS REQUIRED TO TRYOUT FOR AAA AND A SEPARATE FORM IS REQUIRED FOR JR TRYOUTS/JR CAMPS

3. FORMS WILL BE EMAILED TO THE EMAIL ADDRESS PROVIDED WITHIN 72 HOURS. PLEASE CHECK YOUR JUNK MAIL
